The Integrated Day Center (C.D.I.) of Fondazione Paolo VI offers a complete and qualified service for elderly people who need health and social support but wish to continue living at home.
Our goal is to maintain and encourage the social, cultural and recreational life of older adults while providing qualified health, nursing and rehabilitative assistance in a safe and welcoming environment.
The C.D.I. has a maximum capacity of 20 places and is mainly intended for:
- Elderly people (65+) of both sexes with reduced self-sufficiency.
- People under 65 who are unable to live independently.
It gives priority to residents of the Municipality of Bagnolo Mella, especially those in economic, family or social hardship, but may also accept users who do not reside in the Municipality in specific cases.
Upon admission, for the safety of all guests, an antigen test will be performed, repeated weekly as screening.
The nurse will document the result in the clinical diary.
The multidisciplinary team – doctor, nurse, OSS, educator and physiotherapist – will collect medical history, therapy and all personal information to complete the FASAS and develop a provisional care plan.
The personalized care plan will be finalized within 30 days of admission and updated every six months.
